Instructions for Students conducting self-initiated research on human subjects that need to submit an ethics review form

1. Student fills in Student Initiated Ethics Review form
2. Student must sign ethics review form
3. Professor must review and sign ethics review form
4. Submit the completed ethics review form to Josie Alaimo in the Undergraduate Office by the deadline.
